If there are any changes to your placement (e.g. your agency has changed, supervisor change), or you need to withdraw or cancel your placement please submit the Placement Extension / Change form within 7 days of any pending changes.
NOTE: If you are withdrawing after the census date, you will need to apply for a late withdraw here.
