Public Health Service Placements – Mandatory Pre-Placement Requirements

Please note:

  • Store copies of these documents on OneDrive or on personal devices for future reference and use.
  • Take copies of these documents on the first day of orientation and/or placement.
Please complete all the following requirements

MONASH HEALTH

Upload to Sonia Online Provide to Monash Health

At least three weeks prior to placement orientation/start date with Monash Health, students are required to complete all the mandatory tasks outlined on this page.

The following checks need to be completed and uploaded to Checks page on Sonia Online:

  1. Police check:
    1. Police checks are valid for 12 months from the date of issue and must have 6 months validity remaining
    2. Monash health do not accept statutory declarations in place of police checks.
  2. Current Working with Children’s Check (WWC), with an expiry date that is 6 months or more after placement start date
  3. Current AHPRA Registration
  4. Immunisation Declaration. Students are required to consult with an AHPRA registered medical practitioner to have them complete the
    • Health Student Immunisation Declaration to confirm students have the required immunisations for working with Monash Health. Please print a copy of this form and take to your doctor to complete, then scan and upload the completed form to Sonia (Checks page).

Students who are not up to date with their immunisations need to contact the Cairnmillar placement coordinator to discuss options.

The following tasks must be completed and provided to Monash Health: Upon confirmation of your placement, you will receive an orientation email from Monash Health. The email will contain instructions and tasks you need to complete. Generally, you will need to do the following:
  1. Read the email instructions and all documents carefully
  2. Complete all the requirements and the mandatory training modules
  3. Download, complete and store copies of:
    1. Placement contact information and checklist
    2. Confidentiality agreement
    3. Signed student declaration
    4. Mandatory training modules (including downloading course completion records)
  4. Merge all the documents into one PDF file (including any extra documents required in the forms), preferably in the order outlined above.
  5. Save the PDF in OneDrive or safely on a personal device. Cloud based back up is recommended.
  6. Email this PDF document and the completed calendar excel spreadsheet to the Monash Health Placement Coordinator as instructed in the email.

WESTERN HEALTH

At least three weeks prior to placement orientation/start date with Western Health, students are required to complete all the mandatory tasks outlined on this page.

The following checks need to be completed and uploaded to Checks page on Sonia Online:

  1. Police check:
    1. Police checks are valid for 12 months from the date of issue and must have 6 months validity remaining.
    2. Western health do not accept statutory declarations in place of police checks.

  2. Complete all the requirements and the mandatory training modules.
  3. Complete and store copies of:
    1. Student Undertaking form (Schedule 3)
    2. Security Access Request form
    3. Next of Kin form
    4. Mandatory training modules (including downloading course completion records)
  4. Merge all the documents into one PDF file (including any extra documents required in the forms), preferably in the order outlined in the Welcome email.
  5. Save the PDF in OneDrive or safely on a personal device. Cloud based back up is recommended.
  6. Email this PDF document to TBC. Speak to your placement coordinator for guidance.
